By booking or attending an appointment at the Clinic, you agree to be bound by these Terms.

 

A breach or violation of any of the Terms will result in an immediate termination of your Services.

These Terms and Conditions ("Terms") govern the relationship between you ("the Client", "you", "your") and Sculpt Vitae Aesthetics Ltd ("the Clinic", "we", "our", "us") regarding the provision of non-surgical aesthetic treatments (“Procedures”, “Services”) and related services at our clinic.

 

General Conditions

 

1.1 We reserve the right to refuse service to anyone for any reason at any time.

 

1.2 You agree not to reproduce, duplicate, copy, sell, resell or exploit any portion of the website or service, use of the service, or access to the service or any contact on the website through which the service is provided, without express written permission by us.

 

 

2. Appointments and Consultations

 

2.1 Bookings can be made using our website booking link, email, phone or social media messaging. We do not accept walk-ins. 

 

2.2 All Clients must attend an initial face to face consultation before undergoing treatment. This is complimentary and included with all services.


2.3 We reserve the right to refuse treatment if we believe it is not in your best interest or is not medically appropriate.


2.4 Clients must be aged 18 years or over. Proof of age may be required in the form of Government approved identification. Such as a Driving licence or Passport. If you are asked to provide identification then we reserve the right to cancel your treatment. 

 

2.5 In accordance with our professional registration, children or young people under the age of 18 are not permitted to use our website or receive any services from us. Children or young people under the age of 18 are not permitted in the Clinic or in the Clinic building. 

 

2.6 On booking an appointment with us, the Clinic’s full address will be provided via email. As we are an At-home clinic, for safety reasons this is not provided on our website. 

 

3. Medical Disclosure

 

3.1 You are responsible for providing an accurate and complete medical history, including any allergies, medications, pre-existing conditions and previous/ future surgeries via full completion of the Medical History form. 


3.2 Failure to disclose medical and relevant information may result in adverse effects or complications, for which the Clinic including its practitioners, directors, officers and successors will not be held liable.

 

4. Consent

 

4.1 All patients must sign an informed consent form before undergoing any procedures with us. 

 

4.2 You are responsible for reading all treatment literature provided by us before your procedure. 

 

5. Payment and Refund Policy

 

5.1 You must pay us for the services you have secured in accordance with our pricing and no promissory note will be accepted. The total treatment fee is payable upon completion of the service. 

 

5.2 Any user who receives a service from us but fails to pay the full price of the service, where no prior agreement has taken place, will be reported to Police authorities.

 

5.3 A Non-refundable Deposit of 25% of the service fee is required for all procedures. If the booking fee has not paid, the service is not confirmed. Please see 6.2 for more information. 

 

5.4 If the Practitioner does not agree with proceeding with the booked service, they will inform you of the reasons and will refund your Deposit. 

 

5.5 Please note we may alter our prices at any moment without prior notice. Once your booking is secured, any price alterations to our services will not apply to you. 

 

5.6 We shall not be liable to you or to any third-party for any modification, price change, suspension or discontinuation of the Service.

 

5.7 We accept payment via debit/credit card, bank transfer or cash.


5.8 Refunds are not given for completed treatments. In the event of dissatisfaction, we encourage you to contact the Clinic for a review and follow-up consultation. See contact details below. 

 

6. Cancellations and Late Arrivals

 

6.1 A minimum of 24 hours’ notice is required for appointment cancellations or rescheduling.


6.2 Cancellations within 24 hours of your appointment or no-shows will result in the loss of your deposit. This may not be able to be put towards another appointment. All decisions are subject to our discretion. 

6.3 Late arrivals may result in a shortened treatment or rescheduling, depending on our availability.

 

6.4 If you need to cancel or reschedule your appointment and provide appropriate notice (greater than 24 hours before the appointment time) the deposit can be refunded or put towards a rescheduled service. Please contact the Clinic in order to cancel or reschedule any appointment. 

 

6.5 If a Client does not attend or consistently attend for a booked appointment(s), they may be banned from further bookings with the Clinic. This is at the Clinic’s discretion. 

 

7. Treatment Results

 

7.1 Results from aesthetic treatments can vary based on individual response and compliance with aftercare advice.


7.2 The Clinic makes no guarantee as to the precise outcome of any treatment.


7.3 You acknowledge that aesthetic treatments are elective and results are not permanent.

 

7.4 All patients should have realistic expectations regarding post- treatment outcomes. This will be discussed during consultation. Any questions, please contact the Clinic.

 

8. Aftercare and Follow-Up

 

8.1 You will be provided with specific aftercare instructions following treatment.


8.2 It is your responsibility to follow all aftercare guidance to minimise the risk of complications.


8.3 Follow-up appointments may be recommended and may be subject to additional charges unless otherwise stated- see 8.4.

 

8.4 All Anti-wrinkle treatments must engage in a complimentary post treatment face-to- face review as routine. If on review, further Anti-wrinkle treatment is required during this appointment, this will be complimentary and all further treatment is subject to our Professional discretion. 

 

9. Liability

 

9.1 The Clinic carries appropriate professional indemnity insurance as required under English and Welsh law.


9.2 We are not liable for any indirect, incidental, or consequential damages that may result from treatments, including dissatisfaction with results, unless due to negligence.

 

 

10. Privacy and Data Protection

 

10.1 All personal data is processed in accordance with the UK General Data Protection Regulation (UK GDPR) and the Data Protection Act 2018.


10.2 Your data will only be used for the purpose of providing you with safe and effective treatment, this includes secure storage of consent forms, medical history forms and before and after pictures for Professional insurance purposes. This information will be stored securely on a patient management system- namely www.Facesconsent.com. Please see their terms and conditions and policies for more information.  

 

10.3 A Client’s image/voice may be shared for promotional and/or social media purposes only if written consent is provided. This may be shared as video, photograph, audio or other media now known or later developed. 

 

10.4 Client images will not be shared without your explicit consent unless required by law or other valid authority.

11. Complaints

 

11.1 We aim to provide the highest level of care. If you are dissatisfied, please raise your concerns in writing to: Info@sculptvitae.co.uk


11.2 We will aim to acknowledge complaints within 5 working days and will provide a full response within 20 working days.

 

12. Changes to Terms

 

12.1 The Clinic reserves the right to amend these Terms at any time.


12.2 Any updates will be posted on our website. Your use of our services constitutes acceptance of the revised Terms.

 

13. Governing Law

 

13.1 These Terms are governed by the laws of England and Wales, and any disputes shall be subject to the exclusive jurisdiction of the courts of England and Wales.

 

14. Other

 

14.1 You shall not disclose the address of the Clinic for any reason. 

 

14.2 By booking a service, you confirm that you have read, understood and comply with our Terms and Policies. 

14.3 A Client can be banned or asked to leave the Clinic without any explanation; this is completely at the Clinic’s discretion. 

 

14.4 All content related to Sculpt Vitae Aesthetics (logo etc.) are exclusive property of Sculpt Vitae Aesthetics Ltd and ‘All rights are reserved’.

 

15. Unexpected Circumstances

 

15.1 In certain circumstances, appointments may need to be cancelled or rescheduled by us due to unexpected circumstances beyond our control. In this case, we will aim to provide as much notice as possible and will work with you to reschedule the appointment. If no appropriate date is found, then the Deposit will be refunded.

 

15.2 We will not be liable for any unexpected circumstances or events that may lead to your appointment being cancelled or rescheduled. 

 

15.3 We are not responsible for any failure to perform due to circumstances beyond our control (e.g., natural disasters, pandemics).

 

16. Third Party 

 

16.1 Our website may contain links to third party websites or services that we do not own or control. We are not responsible for the content, policies or practices of any third-party website or service linked to on our site. It is your responsibility to read the terms and conditions and policies of these third-party websites before using these sites.
